Evidence: On 01/30/2023 staff could not explain how child A (Age 4) woke up from nap with a round, blistered mark on their right nostril. Staff confirm that child A did not have a mark on their face before nap. Staff were not able to provide an explanation as to how Child A sustained the injury to their nose. Child A did not articulate how the mark came to appear on their nose.
