Video footage shows Staff A and Staff B on-and-off their cell phones multiple times, with neither staff supervising the children on the play equipment.

In addition, the door is not able to be completely closed which caused a gap around three sides of the door. The door has been broken for at least the last three weeks at the time of inspection.
