Staff #1 confirmed that on 1/11/2023 at approximately 5:10 pm, Child #1 (age 2-years) was served approximately 4 crackers for snack, exposing Child #1 to an allergen. Staff #1 did not verify Child #1?s allergens listed on the center allergy list before serving Child #1 crackers. Child #1 suffered a severe allergic reaction.
