On March 15, 2024, Inspector viewed video camera footage of an incident recorded on February 15, 2024 that showed Staff 2 present in the classroom when Child #1 (three-year-old) was hit with Staff 1?s croc shoe. 2. Staff 2 was interviewed and confirmed that they did not report the incident to administration. 3. The center administration staff also stated they did not contact CPS

8VAC20-780-40-E · Based on observation and interviews, staff did not follow operational responsibilities of the licensee by ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures.

Evidence: 1. Staff did not follow the center's policies and procedures that Infants, who cannot turn themselves over and are awake shall be placed on their stomachs a total of 30 minutes each day to facilitate upper body strength and to address misshapen head concerns. Time is to be broken up into 2 or 3 times a day to obtain that total amount per day. 2. In review of 9 infant daily communication forms, staff did not document 30 mins and 15 minutes of tummy time on the form. Staff did document bottle feeding and diaper changing.

8VAC20-780-270-A · Evidence: Observation of the classroom of three year old children located on the lower level of the center had a water leak which had odor that could be smelled as you enter the center. The carpet in the classroom was wet due to the water leak. Upon arrival in the classroom, the windows were closed upon entry in the classroom and there was no equipment use to help with the odor in the classroom.
