8VAC20-780-40-E · The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

On 6/2/2026, Staff A, Staff B, and Staff C did not maintain compliance with the center?s policy and procedure for identifying where attending children are at all times. Staff did not account for children and their transition location on the center?s attendance accountability checklist. Periodic name-to-face checks were not completed. A visual check of the playground area was not completed before leaving to go inside the center. Child A, a six-year-old, and Child B, a seven-year-old, were left outside on the playground, alone and unsupervised. While the children were unsupervised on the outdoor playground, they climbed over the playground fence and walked to the front entrance of the building to ring the doorbell to return to the center.
