8VAC20-780-340-F · Based on interviews, the center failed to ensure that all children under 10 years of age shall be within actual sight and sound supervision of staff.
Evidence: Evidence: 1. Staff #2 stated that the parent of Child #1, age 5, called at approximately 4:10PM and let Staff #2 know that they were arriving to pick up Child #1. Staff #2 walked to the opposite end of the gym to get the sign out clipboard and did not see Child #1 leave the gymnasium. Staff #2 did not know Child #1 was gone until Staff #1 let them know that Child #1 had left the gym and exited the school across the hall from the gym. Staff #2 exited the school and Child #1 was standing at the curb approximately 25 feet from the school exit as their parent pulled up. Staff #2 stated that Child #1 was out of sight and sound supervision for approximately 1 minute. 2. Staff #1 stated they were in the hallway with another child in the restroom when they saw Child #1 exit the school across the hall from the gym. Staff #1 stated they informed Staff #2 that Child #1 had exited the school building.
