8VAC20-780-40-E · Based on review of the center?s policies and procedures and interviews, the center did not ensure that the center?s activities and services were in compliance with the center?s own policies and procedures related to supervision that are required by the standards.
Evidence: 1) During interviews, staff #1 reported that on 11/04/2022, during the transition from the classroom to the outdoors, Child #1, two-years old, was mistakenly left in the classroom unsupervised for approximately 15 minutes. Staff #1 reported that a name-to-face check was not completed upon exiting the classroom. Staff #1, staff #2, Staff #3 and staff #4 were unaware that Child #1 was left behind in the classroom. Staff #1 reported that at approximately 10:35 am, the parent of child #1 texted staff #1 asking if child #1 was ok as she did not see him in any photos that were being sent out of the nature walk. Staff #3 returned to the classroom where child #1 was waiting by the door. 2) The center?s Child Supervision Policy stated, ?Classroom teachers are responsible for the supervision and whereabouts of the children assigned to their care at all times, which entails conducting regular headcounts. Name to face head counts are to be done anytime the class is leaving the room to go outside to the playground, once on the playground and going on field trips to ensure no child is hiding or left behind. ?
