Based on review of video footage on 11/20/2025, Teacher # 1 was observed picking up Child #1 (4 months old), who was buckled in an infant bouncer chair. Teacher #1 picked up the bouncer chair with Child# 1 still buckled in and began walking across the room. Teacher #1 tripped over a toy and fell, dropping the bouncer with Child #1, to the ground. The bouncer chair landed hard onto the ground, back into a stationary position. Child #1's head can be seen in video footage bouncing up and down off the bouncy seat, and his entire body lifting up from the bouncy seat when the chair landed. Warning label on the back of the infant bouncer chair read "NEVER lift or carry baby in bouncer".

On 10/25/2024 Staff # 1 and Staff #2, did not step in to address the parent of Child #1 who, walked from her car, up to the bus and signaled Child #2 to come speak with her. Child #1?s parent was bending down to Child #2?s eye-level and speaking to the child, in response to a matter that transpired on the bus from the public school to the facility between Child #1 and Child #2. Staff #1 and Staff #2 did not intervene, and Child #2 was visibly upset and crying during the interaction with Child #1?s parent.
