The screws on the seat of the wooden picnic table on the playground were loose allowing the seat to move up and down creating a pinch point. Children were observed using the table during the inspection. 2. The wooden siding of the building, in the playground area, is deteriorating in several areas and exposes the nails and screws. 3. The molding around the flooring in the school-age classroom is loose in two areas and creates a tripping hazard. A child was observed tripping and falling over the loose molding during the inspection.

The center's Discipline Policy states ?Corporal punishment, which includes: spanking, shaking, and slapping is prohibited. The carpeted areas in the Pre-K classroom were littered with dirt and various debris/trash; the carpets appeared to have not been vacuumed. The children were observed sitting on these carpets. 4. The toilet in the restroom of the 2 ?-3 year old classroom had dried feces around the inside of the toilet. 5. The floors of the restroom in the Pre-K classes were littered with various debris/trash and dirt. There was a dirty/used clear plastic cup laying in the floor of the restroom.
