8VAC20-780-340-F · Based on interviews, the center failed to ensure that all children shall be within sight and sound supervision of staff.
Evidence: 1. Witness #1 stated that Child #1, age 8, left the cafeteria with Child #2, age 5, to go to the bathroom. No staff were present. Child #1 came out of the bathroom alone and walked back towards the cafeteria. Child #1 wanders and walks up and down the stairs and eventually enters the library where a school teacher speaks to the child. Child #1 is brought back to the cafeteria. Child #1 was out of sight and sound supervision for approximately 4 minutes. Child #2 came back a couple of minutes after Child #1 and was out of sight and sound supervision for approximately 6 minutes. 2. Staff #1 stated they were near the cafeteria kitchen entrance and did not see the children leave the cafeteria. They did not know they were gone until the school teacher brought back Child #1. 3. Staff #2 stated they were on the stage gathering kids to wash hands and did not see either child leave.
