Multiple rest mats within the sites classrooms were observed to be punctured in a manner where the cushion was exposed. Since they are in bad repair, the rest mats are not able to be properly sanitized.

In the children?s bathrooms in the cafeteria area, there was an electrical switch that was missing one of the two switches. This is a hazard to children. Also, in the bathroom the light fixtures above the sink had the light bulbs exposed without a covering, this is a hazard to children. Children were observed in this area.
