Evidence: Staff #2 stated occasionally a staff member would monitor and walk between two separate classrooms when the center was short staffed. Children would be asleep in both classrooms.

The record did not contain documentation of any further contact with the Office of Background Investigations within 30 days of employment, and the staff member had been continuously employed.

22VAC40-665-810-D-1 · Based on interviews and documentation review, the vendor transported children that were not properly secured in a child restraint device of a type which meets standards adopted by the United States Department of Transportation.

Evidence: A member of management stated during the virtual inspection, children younger than eight years of age were being transported on a van to and from public school. On 10/20/2021, another member of management stated the center borrowed two vans for transportation and the booster seats were left in the original vans.
