When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. The medication authorization for child #16 expired two months ago. The medication was on site.

22.1-289.035-B-4 · Based on the review of one staff record (Staff #4), and an interview with a center representative (CR #1), the center failed to obtain a copy of the results of a search of the child abuse and neglect registry from any state where the individual has resided in the preceding five (5) years.

Evidence: The staff record contained no documentation of Out of State Child Abuse and Neglect results from Massachusetts for Staff #4. Staff #4 began employment on 8/21/23. The sworn statement dated 7/08/23 documented that Staff #4 had resided in Massachusetts within the last 5 years. In an exit interview on 5/13/24, CR #1 acknowledged that there was no documentation of Out of State Child Abuse and Neglect results from Massachusetts for Staff #4.

Evidence: 1. The centers policies and procedures states ?RRP operates below child ratios in all age groups, with two teachers assigned to each class at all times, with the exception of nap. " 2. The licensing inspector entered room #2, staff #11 was alone with 15 children. Staff #11 stated that staff #7 went to get an outlet plug from another room. Staff to children ratio requirements for class #2 is 1:10 whenever children are in care.
