The record for Child #4 (first date of attendance 06/21/2022) contained documentation of immunizations dated 06/28/2022. 3. Staff #1 confirmed this information during the inspection.

Evidence: 1. During the inspection, the licensing inspector reviewed information regarding the center that is disclosed to parents or guardians in writing and made available to the general public. 2. The written information did not include enrollment capacity, health requirements and public liability insurance. 3. Staff #1 stated she was not aware this information needed to be included in the parent handbook.
