1. The infant classroom had a bottle of bleach cleaner, and other disinfectant in a cabinet that had one door unlocked and within reach of children. 2. The 3-4's classroom had bleach and sanitizer on the floor, in an unlocked closet, within reach of children.

Evidence: During an interview with staff, the staff described administering teething gel to an infant that was provided by the child?s parent for teething pain that morning. Licensing inspector reviewed the child?s record and did not observe a written authorization from this child?s parent to administer this nonprescription medication as required by 8VAC20-780-510.B.2

Evidence: 1. The record of Staff #1 (DOH 7/6/21) did not contain documentation of results of an out-of-state sex offender registry check. 2. Administration acknowledged the sex offender registry was not completed.
