8VAC20-780-500-A · Based on observation, the center failed to ensure that staff shall wash their hands with soap and running water before and after diapering and children's hands shall be washed with soap and running water after a diaper change.

Evidence: A staff member in the infant classroom was observed diapering a child. Staff did not wash their hands or the child?s hands after diapering. After diapering the child, staff placed the child on the floor and staff picked up several other objects in the classroom.

The director stated that she would contact the owner and have him hammer the nails down. The director stated that children will not use the stairs until the repairs have been made. 3. The wall in the kitchen next to the door, has a large hole in the wall about 5 ? - 6 inches stuffed with plastic bags. 4. The director said that she was aware of the hole.

Evidence: 1 Two staff interviewed stated that a mouse was seen both alive (running through the infant classroom) and dead (caught on a sticky trap in the classroom area. 2. Staff stated that the mice had been noticed at the center for over a month. 3. The center director confirmed that sticky traps placed around the center, when the children are not in care. Sticky traps are taken up prior to the arrival of the children. 4. The health department observed rodent feces and debris behind the freezer and near the compartment sink in the kitchen per the inspection report on 11/4/2022. A violation was cited by the health department.

Evidence: During the inspection on 04/28/22, the following was observed: 1) An electrical outlet in an upstairs room was observed with no face plate covering, exposing wires and peeling paint. This was accessible to the children. 2) A doorknob in the June Bugs classroom was lose, creating a pinching hazard. 3) One of the picnic tables located on the preschool playground was damaged with deteriorating wood and a crack in the middle of the table top. It was reported the children use the table. 4) Two PVC pipes were observed protruding out of the ground on the side of the playground where the children play, creating a tripping hazard. It was reported two tables were removed and the umbrella stands were exposed. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

The copy of the the results of a criminal history record information check and a sex offender registry check are required to be obtained prior to hire. 3) During interview, a member of management reported the required out-of-state background checks for Staff #2 and Staff #4 had not been requested.
