The references were missing the signature of the person making the call. 3) The references in the record for Staff #4, hired on 05/25/2021, did not contain the required components for references that were taken over the phone. The references were missing the signature of the person making the call. Staff records should contain documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.     8VAC20-780-270-A · Based on observations and interview, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.

    Evidence: During the inspection on 04/28/22, the following was observed: 1) An electrical outlet in an upstairs room was observed with no face plate covering, exposing wires and peeling paint. This was accessible to the children. 2) A doorknob in the June Bugs classroom was lose, creating a pinching hazard. 3) One of the picnic tables located on the preschool playground was damaged with deteriorating wood and a crack in the middle of the table top. It was reported the children use the table. 4) Two PVC pipes were observed protruding out of the ground on the side of the playground where the children play, creating a tripping hazard. It was reported two tables were removed and the umbrella stands were exposed. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
