Evidence: The bathroom flooring in the early pre-school and pre-school rooms was observed to cracked and pulling away from the concrete floor creating a trip hazard for children and staff. Ceiling tiles in both bathrooms are broken and coming out of the metal framing track.

8VAC20-780-340-F · Based on interview and documentation the facility failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.

Evidence: According to interview, C2 age 2 was left alone on the playground on 08/08/2022 for approximately 22 minutes and the classroom teachers did not know that the child was left alone on the playground until the parent of the child reported seeing the child alone on the playground. The approximate temperature at 11:00 a.m. on 08/08/2022 was 85 degrees Fahrenheit. The incident report completed by the center documented that ?during the transition from the playground to the classroom C2 was separated from the group causing the child to remain on the playground. The report documented that actions that will be taken to prevent the incident in the future to include ?proper face to name procedures, checking playground?.

Evidence: 1. In the pre-school classroom, the bottom of the sink was observed to be broken creating sharp edges and there were protruding nails that could snag children's clothing or skin. 2. Bus #1 had a hole in the seat approximately an inch in circumference which exposed the inner foam material. 3. Bus #3 had a hole approximately 10x5 which exposed the inner foam and seat material. The foam was ripped and missing in several areas. 4. Administration acknowledged the broken sink and the holes in the bus seats.
