The record of staff #2 contained documentation of a tuberculosis screening dated one month after the date of employment. 3. The record of staff #3 contained documentation of a staff health form dated 20 days after the date of employment.

22.1-289.031-B-5 · The center shall establish and implement procedures for ensuring that all areas of the premises accessible to children are free of obvious injury hazards. Classroom #3, Classroom #4 and Classroom #8 had electrical outlets that did not have a protective cover. Children were present in the classrooms. Classroom #5 contained four containers of cleaning chemicals sitting on top of a cabinet. The cleaning chemical containers were unlocked. A trampoline and a riding toy located on the Toddlers playground were torn, creating sharp edges and entanglement hazards.
