Evidence: 1) The fingerprint-based national criminal background check in the record of Staff #3, employed on 09/08/20, was completed on 05/19/20. The facility must not accept a required background check that is dated more than 90 days prior to employment. 2) The fingerprint-based national criminal background check in the record of Staff #5, employed on 09/08/20, was completed on 05/22/20. The facility must not accept a required background check that is dated more than 90 days prior to employment. 3) The fingerprint-based national criminal background check in the record of Staff #6, employed on 08/29/22, was completed on 03/02/22. The facility must not accept a required background check that is dated more than 90 days prior to employment. 4) The fingerprint-based national criminal background check in the record of Staff #7, employed on 09/13/21, was completed on 03/09/21. The facility must not accept a required background check that is dated more than 90 days prior to employment. 5) The fingerprint-based national criminal background check in the record of Staff #9, employed on 11/01/19, was completed on 11/08/19. 6) During interview, a member of management confirmed the fingerprints were not obtained within the required timeframe.
