    center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. Based on review of the center?s playground policy, it states that "staff will position themselves at all times during playground activities so that children are within staffs? sight at all times. During staff interviews on 5/12/2026, it was confirmed by Staff #1 and Staff #2 that during the playground activity, both staff members were positioned on the same side of the playground and were unable to observe Child #1 for a few moments. As a result, staff did not witness Child #1 fall, which subsequently resulted in a fractured arm.
