The center?s playground safety procedure states that ?All staff members are to distribute themselves in an appropriate manner, evenly throughout the playground, so that all children may be seen at all times. If there is more than one classroom outside, remember that you are responsible for the direct supervision of all children and not simply for the class you lead.? Staff #1 acknowledged that she lost sight of child #1 sometime in between lining up and coming inside. Staff #2 stated that she did not see child #1 after staff #1?s class departed to go inside the building. 2. Administration acknowledged that the staff did not follow the safety procedures for active supervision that are required by the standards.
