Evidence: The records for Child #1 (first day of attendance 04/12/2022) and Child #6 (first day of attendance 01/30/2023) did not contain written authorization for emergency medical care should an emergency occur, and the parent cannot be located immediately unless the parent presents a written objection for the provision of medical treatment on religious or other grounds. The records did not contain a written statement that the vendor will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so, requested by the vendor.

Evidence: 1. During the inspection, Caregiver #1 could not located the background checks for Household Member #1. 2. Caregiver #1 stated Household Member #1 has lived in the house for years and required background checks for Household Member #1 had been obtained. 3. The record for the vendor maintained by the Virginia Department of Education did not contain information regarding background checks for Household Member #1.

Evidence: The preliminary information gathered for the remote inspection indicated Household Member #1 was residing in the home. The provider stated Household Member #1 moved into the home in March 2021. The vendor stated a fingerprint based background check, findings of a central registry search and a sworn statement had not been obtained for Household Member #1 as of 08/04/2021.
