Evidence: Interview with Director, Curriculum Specialist, Teacher, and Parent, and documentation review, revealed that there were behavioral problems identified with child #1 that caused concern. The child was observed over a four day period prior to the parents being made aware of the behavioral concerns.

    Evidence: 1) Email dated 09/14/2022, sent out to all parents stated that some of the COVID restrictions that had been in place for this school had been lifted on August 1, 2022. These include parents being able to schedule volunteer time inside the school and scheduling a birthday celebration. 2) Email dated June 24, 2022 states ?since we are still (unfortunately) in the midst of COVID, we are currently only allowing staff and students in the building.? 3) When asked on September 22, 2022, the Director stated that parents are not allowed to enter the building without scheduling an appointment first.

    8VAC20-780-340-A · Based on interview, the center failed to ensure when staff are supervising children, they always ensure their care, protection, and guidance. Evidence: When interviewed administration stated that child #1 was left in the care of the center on 9/22/22. Child #1, 4 years old, was left on a stairwell by staff #3 who escorted the child to the bottom of the stairs and turned around and walked away. The stairs are located on the outside of the building. Child #1 was left alone for approximately 15 minutes. The center is located in an urban area surrounded by busy streets with heavy traffic.
