    8VAC20-780-270-A · Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

    In Room B, near the bottom portion of the green and brown painted wall, the horizontal metal strip was observed to be detaching and not fully secured to the wall. The paint in this area was chipped and peeling in several spots. The lower section of the wall also had a damaged section of trim, leaving the drywall exposed. In Room C, toward the back of the room, the lower section of the interior wall had a long horizontal line of dark staining along the seam between drywall panels, as well as exposed yellow expanding foam protruding from the seam. Additional dark discoloration was present near the corner bracket, and peeling paint was observed on both the wall surface and the attached metal bracket. In Restroom #3, behind the first toilet, the lower wall had a circular metal plate that was not fully secured, leaving an opening that exposed damaged wall material. Behind the second toilet, there was an opening in the lower wall with exposed wall material where the water supply line entered the wall.

On the playground, there were three types of playing equipment with rust and chipping paint: the blue spin-around unicycle, blue hummer metal car and a green, red and blue jumping toy. A pink push-car had a broken steering part presenting sharp ridges, and a piece of the playhouse was laying on the ground, and was cracked and had exposed nails.

    On 10/11/2024, during a field trip to a local park, Child #1 (aged 3 years old) was out of sight and supervision for approximately 10 minutes. Child #1 was found by a community member, who was not staff, and returned the child to Staff #1, and Staff #2. In the surrounding area, there were two roads, a parking lot and a pond that was accessible and within walking distance to the playground.

    Evidence: During the November 10, 2022 inspection, the following was documented: Upon the licensing inspector and licensing administrator?s arrival at the center, a person later identified as staff #1 was observed opening the front door of the center for the postal courier. The two licensing agents approached the door at 3:00 pm and knocked multiple times and rang the doorbell without a response from the center. While one of the licensing agents was on the phone with the local police department, the owner arrived at 3:18 pm and proceeded to allow the agents into the center.
