Evidence: A. Information required to be provided in writing to parents prior to the child's first day of attendance did not include the following: 1. The center's policies for the arrival and departure of children, including procedures for verifying that only persons authorized by the parent are allowed to pick up the child, picking up children after closing, when a child is not picked up for emergency situations including but not limited to inclement weather or natural disasters; 2. Description of established lines of authority for staff; 3. Policy for reporting child abuse and neglect as required by The Code of Virginia; 4. Discipline policies including acceptable and unacceptable discipline measures. B. The following required written policies for staff were not addressed in the policies and procedure manual; 1. Procedures for Children Arriving Late; 2. Procedures for Absent Child (when child attends another program); 3. Procedures for Identifying Where Children Are at All Times; 4. Procedures for Ill, and Injured Children; 5. Procedures for Medication Administration; 6. Safety Policies for Swimming/Wading; 7. Job responsibilities and to whom they report; and 8. Confidential treatment of personal information about children in care and their families.

In addition, the facility shall be required to prominently display next to the posted provisional license a notice with a description of specific violations of licensing standards to be corrected and the deadline for completion of such corrections is available for inspection at the facility and on the facility's website, if applicable.
Evidence: 1. The Licensing Inspector observed the license that was posted expired on March 6, 2024 2. The Provisional license which was issued on March 7, 2024 was not posted in the center.

The director acknowledged the two?s classroom was out of ratio and confirmed no additional staff was available.

Staff #1 was trying to place child #1 in the infant room because he had bit the child of staff #1. Child #1 was crying and trying to avoid going into the Infant room. Staff #1 was observed pushing child #1 away from her and into the infant room stating the child had bit her.

The records for Staff #1 and Staff #4 did not contain documentation of a sex offender registry check and documentation of a request for an out of state child abuse and neglect search. 3. The record of staff #4 did not contain documentation of an out of state criminal history check. 3. A member of management confirmed this information during the inspection. 4. Documentation of an out of state sex offender registry and an out of state criminal history check are required to be obtained prior to being hired. Documentation of a requested for an out of state child abuse and neglect search by the end of the 30th day employment.
