    Evidence: 1. In the school age room, there was a chipped and cracked tile on the floor near the computers. 2. On the playground, there was a blue rainbow climber that was not securely anchored to the ground. When pulling on the structure, the structure began to tilt over. 3. On the playground, there was chipped paint and rust on the bars of a yellow, metal car structure. 4. On the playground, the end cap for the left handlebar of a yellow bike was missing. The rubber covering for the front tire was also missing. 5. On the playground, there was a wooden pole attached to a slab of concrete lying in the grassy area. 6. On the playground, there was a large orange cone that was easy to tip over, in the grassy area, accessible to children. 7. In the infant classroom, there was a crockpot with hot water inside, located on the floor by the refrigerator, accessible to children. 8. In the toddler classroom, there was a hole in the bathroom door, within reach of children.
