The center?s policy on Transitions states that when transitions occur, one staff member should ?make sure that all of the children reach the new area and none are left behind.? 3. On 07/12/2023, at approximately 9:35 a.m. staff and children assigned to the Athletes? Cabin transitioned from Classroom #216 to the gym. Approximately 2 minutes later, Staff #2 entered Classroom #216, finding Child #1 (age: 5 years) sitting at a desk and playing with a flashlight, unsupervised. 4. Staff #1 stated that they called the children by grades, not individually, and did not do a face-to-name check using their checklist prior to transitioning from the classroom to the gym. 5. Staff #1 confirmed that Staff #3 and Staff #4 were in the classroom at the time and neither staff member conducted an accountability or face-to-name check prior to transitioning from the classroom to the gym.
