    8VAC20-780-340-F · Children under 10 years of age always shall be within actual sight and sound supervision of staff.

    On February 25 2026, Child #1 (age 5 years) was not within sight and sound supervision for approximately 4 minutes. Center staff was observed leading a group of children to the school bus area at 8:44:50am. At 8:45:59am Child #1 entered the breezeway of the center. Child #1 could be seen attempting to follow where the group exited, but could not manage the gate. Child #1 was located by a parent at 8:49:15am and was brought to the front lobby of the center.

    8VAC20-780-340-F · Based on interviews, the center did not ensure that all children under the age of 10 years shall always be within sight and sound supervision of staff.

    Evidence: On April 17, 2024 a child (Child #1) was reported to having been left alone in his section of the classroom. It was determined that no other teachers in the other parts of that large room were aware that the child was left unattended. The supervising teacher failed to do a proper name to face check of each child in their care, prior to going outside. Child #1 was left unattended for approximately 3 minutes. There were other teachers near by to hear if Child #1 was in distress, but they were unaware that Child #1 was left in the classroom. Child #1 was located upon another teacher walking through the space heard the child in the room.
