Evidence - 1. On 04/08/2024, at approximately 10:30 am, Child #1 slid down the back of the changing table (from a standing position) on to the floor. Due to an opening in the back of the table, Child #1 sustained scratched to their back. 2. Normally, this table would have its back to a wall, however, the table was moved in order to provide better supervision of all children while staff changed diapers. 3. First Aid was administered to Child #1.

Evidence - 1. On the date of inspection (10/27/2023), the back wall within the "Lizard" room showed signs of damage. 2. The Center Director stated (during an interview conducted on the date of inspection) that this damage occurred when the "ABC" wall was removed leaving several visible spots where the tape had pulled away the top layer of paint and drywall.

8VAC20-780-340-F · Based on interview, (2) two children under 10 years of age were not always within actual sight and sound supervision of staff.
Evidence - 1. The Director stated (during in interview conducted on the date of inspection - 05/19/2022) that on 05/18/2022, starting at approximately 5:21 pm, both Child #1 (4 years of age) and Child #2 (4 years of age) were left unsupervised within a center classroom (Frogs Room). 2. Staff #1 was supervising 4 preschool children on the playground. Two of these children needed to use the restroom. Staff #1 knocked on the fire-door of the Frogs room for the children to come inside to use their restroom. Staff #2 opened the door for Staff #1. Staff #2 then left to go to another classroom. Staff #1 attempted to provide supervision for the 2 children still outside and the 2 children now inside by standing in the threshold of the door. Staff #1 then allowed the door to shut with Staff #1 on the outside. 3. Child #1 was observed by Staff #3 alone in the Frogs Room (Child #2 was using the restroom). Staff #3 took Child #1 up to the front desk to Staff #4. Both Staff #3 and Staff #4 returned to the Frogs Room and found Child #2. 4. Based on video tape, the total amount of time that Child #1 was out of actual sight and sound supervision was 27 seconds. The total amount of time that Child #2 was out of actual sight and sound supervision was 45 seconds. 5. This incident was self-reported to the Office of Child Care Health and Safety (Fairfax).

8VAC20-780-40-E · Based on interview, the center did not ensure that the center's activities, services were maintained in compliance with the center's own policies and procedures (that are required by these standards, and the terms of the current license issued by the department).
Evidence - 1. The Center Director made a self-report to the Office of Child Care Health and Safety (Fairfax) of an incident that occurred on 02/22/2022 at approximately 10:15 am. 2. The Center Director stated (during an interview conducted on the date of inspection - 02/22/2022) that Child #1 (a four year-old) was left unsupervised in a classroom inside the center for approximately 1 minute. 3. It was also reported, that Staff #1 failed to follow center procedure by neglecting to administer a name-to-face roll call while transitioning out of the classroom. Center staff were instructed on name-to-face procedures during a training conducted (the day before) on 02/21/2022. 4. Once Staff #1 realized Child #1 was missing from the group, Child #1 was found in classroom hiding and unharmed. Child #1 was then returned to the rest of the group.
