    Each person in a supervisory position must be certified by a practicing physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision. 1. Staff # 5, employed for 2 days, did not have a staff health form present at the time of the inspection. Staff #5 was observed working in a classroom with children. 2. Staff #10, employed for 8 months, did not have a staff health form present at the time of the inspection. Staff #10 drives the children to and from school and is alone with the children while on the van. 3. Staff # 12, employed for 21 months, did not have a staff health form present at the time of the inspection. Staff #12 drives the children to and from school and is alone with the children while on the van. 4. Staff #14, employed for over 4 years, did not have a staff health form present at the time of the inspection. Staff #14 is the substitute van driver assigned to drive the children when the permanent driver is out.

    22.1-289.031-B-2 · The facility shall establish and implement a procedure for appropriate supervision of all children in care to ensure safety of children . S1 while placing a child into a highchair in the Infant II classroom, C1, an infant pulled himself up on the mini refrigerator in the same classroom, and ??grabbed the crockpot cord and yanked it down causing hot water to fall onto him?. C1 received 2nd degree burns to his face, neck, and shoulder requiring hospitalization. The crock pot was located on top of a mini refrigerator and the power cord was accessible to the child.

    22.1-289.031-A-3-c · Based on observation and interview, the center did not ensure when a group of children

    receiving care included children from different age brackets, the age of the youngest child in the group was used to determine the staff-to-children ratio that applies to that group. Evidence: 1. At approximately 10:00am, the licensing inspector observed 17 children with two staff members in one room. The children?s ages ranged from two years to four years of age. 2. Approximately five minutes later, eight children and one staff member went to the Two?s classroom. Nine children remained in the classroom with one staff member. Child #1 remained with the nine children. Child #1 was two years of age. The other eight children were three years or older. 3. A member of management confirmed Child #1 was two years old. 4. The required staff-to -child ratio is one staff member for every eight children present.
