The center?s policy for supervision of children states ?All individual and/or groups of children must be supervised at all times while in our care?children must not be left alone or unsupervised in classrooms, hallways, offices, lobbies, bathrooms, playgrounds or vehicles?. The center?s policy for playground supervision further states ?Children must be counted name-to-face using the CSR (child supervision record) every half hour and during transition to and from the classroom and the playground?. a. It was reported that on 9/12/23, child 1, age 18 months, was left alone on the playground without staff supervision for approximately 5 minutes. b. The staff members responsible for the direct care of child 1, confirmed that they did not follow the center?s policies for supervision of children, nor did they follow the center?s policy for playground supervision and the use of the Child Supervision Record (CSR).

8VAC20-780-550-P · Based on staff interviews and record review, the center failed to maintain a written record of children's serious and minor injuries.
Evidence: 1. On the morning of 10/7/22, child 1 (age 20 months) was observed crying in the toddler classroom by numerous center staff. a. Staff working in the toddler classroom stated that they did not know why child 1 was crying. b. Staff working in the toddler classroom stated they did not witness any accident that would cause an injury to the child. 2. Five staff who were working at the center on 10/7/22 stated in interviews that they observed child 1 to cry whenever they moved or touched her right arm. 3. Staff 1 and staff 2 stated they took child 1 to staff 3 (center director) because they thought something was wrong with her arm and that she had somehow been hurt. 4. Staff 3 stated she did not observe any injury to child 1 but that child 1 was crying the entire time she examined her. Staff 3 stated that she examined child 1 and moved both of her arms up and down and checked her back. 5. Because child 1 continued to cry, staff 3 stated she called child 1's parent and asked him to pick her up and take her to the doctor. 6. Staff 3 stated that no one wrote an injury report even though numerous staff had observed child 1 to cry whenever her right arm was moved. 7. Child 1?s parent reported to center management that the child had nursemaid?s elbow.
