Evidence: 1. In the outdoor playground area, the 4 plastic toy bins, in reach of children, had cracked and broken lids which poses as a pinching hazard to the children in care. 2. Staff #1 confirmed 4 plastic toy bins were cracked and broken. The center responded with the following: Staff #1 will remove the broken toy bin lids today.

Evidence: 1. Staff #2 (date of hire 7/17/20) has an expiry date of June 2022 for their CPR and First Aid certificate. 2. Staff #1 confirmed that Staff #2 has an expired CPR and First Aid certificate.

Evidence: 1. There was a aerosol can of Lysol sitting on top of the shelf by the entry way to the facility. 2. There was a aerosol can of Lysol sitting on the console at the front of each van used for transporting children to and from each school. 3. Staff #5 (Program director) confirmed there were hazardous substances present that were not kept in a locked place.
