8VAC20-850-30-C-2-b · Health information shall be submitted on the family day provider applicant, assistant and substitute providers, if any, and any other adult household member who comes in contact with children or handles food served to children. An individual who had a nonsignificant (negative) reaction to an initial tuberculin (TB) skin test shall obtain additional screening every two years thereafter. Household Member #1 negative TB screening was overdue by three months.

This training shall be related to child safety, child development, health and safety in the family day home environment, and any required department sponsored training. The provider stated 16 hours of annual training has not been completed by 10/14/2025.
