1. A plastic child size picnic table on the playground was cracked and missing pieces along the seat. 2. The door frame in the toddler room had exposed chipped drywall accessible to children. 3. A metal carpet strip in the drama playroom was not attached to the floor and could be moved around exposing a sharp edge and posing a tripping hazard. 4. The toilet in the rear bathroom has a connecting bolt that rises above the floor approximately two and a half inches. The bolt did not have a cap on top and posed a hazard of puncturing a child in care. 5. In the infant room, five cribs, all assigned to children in care as young as four months, had oversized and loose-fitting crib sheets on them. The material of the oversized sheets was bunched up and was not tightfitting to meet safe infant sleep requirements.

The director stated that the two-year child had been placed in the infant room to nap and that no additional staff had been added.
