A portion of the fence was no longer upright and was on it's side and the top support pole was on the ground. The fence is a tripping hazard and hazard to snagging/cutting children's clothes or skin.

Based on observation and interview, the center failed to establish and implement a procedure to ensure that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining cushioning material under playground equipment. Evidence: 1. In the 3 year old classroom there were three containers of disinfectant wipes on top of the sink accessible to children. 2. The playground contained a painted wooden car with chipped and peeling paint. 3. In two play areas there were two climbing geo-domes over grass with no cushioning material underneath. 4. In the 3yr old classroom in Little Gate and in the 3-4yr old classroom in New Gate there was chipped and peeling paint on the wall.
