Evidence: 1. Diapering was observed in the two year old classroom. The lids on the diaper disposal systems were not connected. Staff was using her hands to move the lid to dispose of diaper changing waste. 2. Staff in the two year old room sanitized the changing table with disinfectant but did not first wash the changing surface with soap and water.
