Staff #1 stated that they were playing Four Square with a group of children, when Child #1 (age: 5 years) and Child #2 (age: 6 years) asked to go to the playground. When the game was over, at approximately 4:50 p.m., Staff #1 gathered the group of children playing Four Square and went inside. They stated that they did not do a headcount. 3. Child #1 and Child #2 remained on the playground, unsupervised. 4. Staff #1 stated that approximately five minutes after going inside, Child #1?s parent arrived to pick up the child. When the parent asked where Child #1 was, Staff #1 realized that the children were still outside and went to retrieve Child #1 and Child #2 from the playground. Both children were still playing.
