8VAC20-780-270-A · REPEAT VIOLATION
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment. In the Pre-K classroom, rust was observed on the escutcheon around the water supply line and on the toilet paper holder in the girls? restroom. In the school-aged classroom, rust was observed on the wall-mounted support frame surrounding the sink.

On the toddler playground, two of the classroom doors, that lead to the playground, had chipped and peeling paint. The post on the gate was cracked and broken. Inside of the the green playhouse, the steering wheel was rusting and had peeling paint, and the green tunnel had multiple areas of splintering wood. On the preschool playground, two of the classroom doors, that lead to the playground, had chipped and peeling paint. The latch on the entrance gate was rusting. Chipped and peeling paint was identified on the following playground equipment: on the yellow steering wheel attached to red fire truck, on the steps and climbing bars on the green and tan playground equipment. On the school age playground, there was a broken gate with two exposing nails. One of the posts surrounding the basketball court was broken with pointy edges and the poles supporting the swing set had chipping paint.

8VAC20-780-340-A · When staff are supervising children, they shall always ensure their care, protection and
guidance. Child #1 was held by the hand and around the wrist for 24 minutes by Staff #1, requiring the child to stay near to the staff?s side without being able to engage in any activities. Child #1 can be seen in video footage trying to pull away from Staff #1.

8VAC20-780-470-F · Based on observation, the center failed to ensure that wading pools shall be emptied, sanitized, and stored in a position to keep them clean and dry.
Evidence: 1. On 7/17/2024, three portable wading pools were identified, in which two of the wading pools were filled with water, leaves and debris. No children were present. 2. According to the center?s schedule and staff, water play takes place every Wednesday and had yet to take place for the week. Staff confirmed that the pools were used previously for play, and had not been emptied and cleaned.

Based on a review of staff records, the center did not ensure that subsequent communicable tuberculosis (TB) screenings were conducted at least every two years from the date of the initial screening. Evidence: The record for staff #3 contained an updated TB screening results of 1/30/2024. A TB screening was due on or before 1/21/2024.
