Staff #9, who has been employed for 6 weeks, was not certified by a practicing physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision. 2. Staff #10, who was employed for 8 days, was not certified by a practicing physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision. 3. Staff #11, who was employed for 8 days, was not certified by a practicing physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision.

Staff #5 was observed alone in a classroom supervising 6 children (youngest child was 14 months old). The children were seated at the tables eating lunch.

22.1-289.031-A-9 · Based on observation and interviews, the child day center was not in compliance with safe sleep guidelines recommended by the American Academy of Pediatrics.

Evidence: 1. The inspector observed Child #1 (Infant) sleeping in a mechanical swing with a blanket wrapped around the infant. 2. The center representative, staff #1 and staff #2 stated that child #1 only sleeps in a mechanical swing or a staff member?s arms 3. Staff #1 and staff # 2 both confirmed that child #1 sleeps in a mechanical swing with a blanket during most naps. 4. The American Academy of Pediatrics state that sleep should be avoided in a ?seating device, like a swing? and ?keep loose blankets, pillows, stuffed toys, bumpers, and other soft items out of the sleep space.?

22.1-289.031-A-5-a · Based on observations and review of documentation, the center did not ensure compliance with the terms of the religious exemption issued by the Department of Education.

Evidence: The letter of exemption states that no more than five children under the age of two and one half are allowed at any one time. During the 02/01/2023 inspection, seven infants (six weeks to 16 months) were being cared for by two staff members in the infant Room and eight Toddlers (17 months to 23 moths) were being cared for by two staff members in the toddler Room. The infant and toddler rooms do not have classroom doors that lead directly to the exterior.
