Evidence: 1. The following hazards were on the outside play area a. Razor blade and a hornets nest were observed on the ground. b. Rust on a metal covering over the window of the center. c. Small holes with jagged edges in the plastic fence d. Covered storage area with no door had multiple nails protruding from the ceiling and wall, a rusted metal strainer hanging on the wall and wires hanging from the ceiling and wall. e. The door leading into the building had areas of peeling paint. 2. The plumbing of the urinal in the one of the bathrooms had a rusted metal plate. 3. In the 16 month-2.5 year-old room the step riser had a piece of separate wood and was rough to the touch. There was also floor board separating from the wall.
