    63.2(17)-1716-B-2 · Based on video review and interviews it was determined the center did not ensure that safety of children through supervision.

    Evidence: 1. A complaint was received that alleged child #1 was roughly handled by staff #1 leaving bruises on their arm on 5/19/21. Pictures sent by child #1's parent showed bruising on child #1's arm. The parent reported that the bruising on child #1's arm was observed at bath time and when asked the child said "Staff #1 did it". Child #1 also reported this to other outside agency officials who are involved in this case. 2. Video footage from a camera in the hallway was reviewed from 5/19/21 and showed staff #1 using both her body and hand to forcibly push child #1 through the doorway to the classroom. Staff #1 was observed in the video with their hand on the child's upper arm when pushing the child into the classroom. 3. Text messages sent by staff #1 to the child's parent included statements such as "I went in there to grab him to come into my room" and "I can't handle this type of stress". 4. A review of the staff policies and procedures as well as interview with staff show that staff #1 did not follow the "Give ` em Five" policy with child #1. 5. Staff #1 could not tell parent or inspector how child #1 received the bruises on his arm and their description of the events in the hallway/threshold to the room do not match the video.
