At 8:05 AM, a child was observed getting out of the car and going into the facility without an adult accompanying the child. The car in which the child arrived drove away when the child reached the front door. There was no staff person in the foyer to supervise the arrival of the child. The facility's policy for arrival of the children as stated in the parent handbook requires the parent to accompany the child into the building when arriving.

There was an active water leak dripping from the ceiling in the classroom off of the back hallway. Numerous stained ceiling tiles show evidence of other roof leaks. 2. The gutters on the back of the building are full of leaf and plant litter. Rain water during the inspection was overflowing from the gutter into gullies created from the overflow.

Evidence: Staff 1 stated that in late May there was an incidence where the facility's procedure for verifying that only persons authorized by the parent are allowed to pick up children at the end of the day. Staff 1 stated that a child was allowed to be picked up from the facility by a relative. Facility staff on duty did not require identification of this person, nor did the staff check to ensure that the relative was a person authorized by the parent to take the child.
