At 8:05 AM, a child was observed getting out of the car and going into the facility without an adult accompanying the child. The car in which the child arrived drove away when the child reached the front door. There was no staff person in the foyer to supervise the arrival of the child. The facility's policy for arrival of the children as stated in the parent handbook requires the parent to accompany the child into the building when arriving.

Both the front and back panels of the classroom door to room 2 have pulled loose creating pinch points and splintering. The wall board by the kitchen in the lunch room is peeling with paint flaking. There are areas of poison ivy coming through the back fence of the playground.

Based upon observation, the facility has not ensured that areas, inside and outside, are maintained in a clean and safe condition. Evidence: 1. There was an active water leak dripping from the ceiling in the classroom off of the back hallway. Numerous stained ceiling tiles show evidence of other roof leaks. 2. The gutters on the back of the building are full of leaf and plant litter. Rain water during the inspection was overflowing from the gutter into gullies created from the overflow.

The downspout from the gutters to the ground near the exit door used by the children to access the playground is missing. There is a large puddle of water four inches deep below where the downspout should have been. 2. The gutters on the back of the building are overflowing with leaves and debris.
