Evidence: 1. On August 17, 2023, Child #1 tripped and fell in the classroom causing an injury to their arm. Staff #2, the child?s teacher, immediately informed the director and the parent was called. The parents transported the child to the hospital. Staff #2 completed a written injury report. 2. Staff #1, the director, confirmed that they did not inform a Department of Education (DOE) representative within two business days of the incident. The Injury of a Child Requiring Outside Medical Treatment report to DOE was filled out on August 30, 2023.
