22.1-289.031-A-3 · The child day center employs supervisory personnel according to the following ratio of staff to children: one staff member to four children from ages zero to 16 months.

In an infant room, where two rooms are utilized, there was one staff member supervising five infants. In the other room, there were two teachers supervising nine infants. Both groups/rooms did not follow the required ratio (1:4).

which shall be made accessible to the department?s representative upon request. The child's record shall include telephone number for each custodial parent's place of employment and the address of at least one person designated in case of an emergency and the parent cannot be reached. Evidence: 1. Child #1 (date of enrollment 9/26/22) did not have the work phone for their father. 2. Child #2 (date of enrollment 3/12/24) did not have addresses for their emergency contact 3. Staff #1 confirmed that a work phone number and address for an emergency contact was not in the records.

8VAC20-790-570-1 · Based on record review and interview, it was determined that the center failed to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.

Evidence: 1. On August 17, 2023, Child #1 tripped and fell in the classroom causing an injury to their arm. Staff #2, the child?s teacher, immediately informed the director and the parent was called. The parents transported the child to the hospital. Staff #2 completed a written injury report. 2. Staff #1, the director, confirmed that they did not inform a Department of Education (DOE) representative within two business days of the incident. The Injury of a Child Requiring Outside Medical Treatment report to DOE was filled out on August 30, 2023.

Evidence: 1. Staff #1 and Staff #2 both have their Virginia fingerprints dated May 10, 2018 which expired on May 10, 2023. 2. Director of the RECDC confirmed both Staff #1 and Staff #2's Virginia Fingerprints are expired and report checks were not completed for 2023.
