8VAC20-780-160-A · Based on record review and interview the center failed to ensure that each staff member submit documentation of a negative tuberculosis (TB) screening before having contact with children and within 30 days of date of employment.

Evidence: 1 - The record for Staff #2 contained a TB screening that was dated August 24, 2024. However, the documented date of hire is August 19, 2024. Therefore, the TB screening was not obtained within the required time frames. 2 - Staff #1 confirmed that the documented date of the TB screening for Staff #2 was after the documented date of hire.
