Evidence: 1. Child 1, age four years, was left alone, crying in the first hallway bathroom, to change his clothing after a toileting accident at approximately 4:30 pm on 06/08/2022. a. Staff 1, who was responsible for child 1, stated she left child 1 alone in the bathroom to change his clothing. 2. Staff 1 was in a classroom with 3 children during this time watching a video. The location and distance of this classroom from the first hallway bathroom does not allow for sight or sound supervision. a. This bathroom is an enclosed single bathroom located around the corner and approximately 21 feet from the classroom where staff 1 was. 3. Staff 1 stated she did not provide intermittent sight supervision or assist the child in the bathroom to clean himself up.
