    8VAC20-780-270-A · REPEAT VIOLATION

    Based on observation and interview it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. In the outdoor play area there was: a. loose wooden pieces at ground level under the gazebo area. b. several of the metal bolts that secure the plastic perimeter were protruding 1-3" from the top of the barrier. c. exposed rust on the play structure. d. the gate to the fencing around the space was broken and had peeling paint. 2. Staff confirmed the above were in need of repair during this inspection.
