Evidence: 1. 5 of the 5 staff interviewed confirmed that staff #3 and #5 got in altercation with each other that involved yelling and pushing in front of children in care. 2. 4 of the 5 staff confirmed that they were aware of at least one child that was visibly upset by the altercation. 3.Staff # 3 and #5 reported they felt they needed to apologize to the children for their behavior during this incident.

8VAC20-780-80-B · Based on observation, record review, and interview it was determined the center did not inform the commissioners representative as soon as practicable but not to exceed one working day of the circumstances surrounding a missing child when local authorities have been contacted for help.

Evidence: 1. Child #2 was missing for approximately 20 minutes on 9/14/21. The local police were contacted. The commissioners representative was not notified by the center. 2. Inspector was notified on 9/16/21 by a referring community agency.
