8VAC20-780-340-A · When staff are supervising children, they shall always ensure their care, protection, and guidance. On September 19, 2024, staff #3 was unaware that child #2(age 3 years) had exited the center building and was in a parking lot for approximately a minute unsupervised. On July 3, 2024 the aftercare staff were unaware that child #1 (age 3 years) had left the multi-purpose room and went to another area of the center.

Evidence: 1. The classrooms in the center only had an electronic record of attendance for the groups of children to document arrivals and departures. 2. Staff #1 confirmed that a written hard copy record of attendance as it occurred was not present in the classrooms.

Based on observation, interview and record review, the center did not ensure that the center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan. This list shall be dated and kept confidential in each room or area where children are present. Evidence: 1. The allergies, sensitivities and dietary restrictions written in each room only included those children who were assigned to a particular room. 2. Staff #1 confirmed that the there was not a current list including all children?s allergies, sensitivities and dietary restrictions in each classroom accessible to staff.
