Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. Evidence: Staff #2 did not have a TB test on July 9, 2024 . Staff #2 was counted in ratio on June 13, 2024 confirmed by director. When volunteer is counted in ratio they would be known as a staff member and need to have all required staff requirements.

Evidence: 1- During the inspection on April 9, 2024 there were 2 staff with 11 children from approximatley 9:07am - 9:30am in the infant/toddler room. The ages ranged from infant to toddler age children. 2- The ratio for infant is 1 staff to 4 children from ages zero to 16 months. 3- Confirmed by staff there was only 2 staff and 11 children.
