Based on staff interview and documentation, the licensee did not ensure the center inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention. Evidence: On July 10, 2024, a child #1 was injured and recieved outside medical treatment. The center did not report the injury with in two buisness day. Confirmed by director.

Evidence: An injury occured on June 3, 2024 while the child was under supervision of the center. The center called 911 and child went to the hospital. Person in charge went with the child in the ambulance. The center did not report the injury to licensing inspector/department representive with in two buisness day.

Based on observation and staff interview, the licensee did not ensure children under 10 years of age always shall be with in actual sight and sound supervision of staff. Evidence: In the infant room, a infant was sleeping in a crib. There was no one in sight supervison of the infant. Staff #3 was observed in the classroom next to the infant room. Confirmed by person in charge walking with inspector during a tour of the facility.
