Evidence: On July 10, 2024, a child #1 was injured and recieved outside medical treatment. The center did not report the injury with in two buisness day. Confirmed by director.

Evidence: In the infant room, a infant was sleeping in a crib. There was no one in sight supervison of the infant. Staff #3 was observed in the classroom next to the infant room. Confirmed by person in charge walking with inspector during a tour of the facility.
